Recently, Panzhihua West District Government Network released the Assessment Report on the Implementation of Responsibility Investigation and Rectification Measures for the "2.9" Object Strike on General Production Safety Accidents in the High-tech Branch of Chengdu Zhongcai Xinjiahao Construction Engineering Co., Ltd. The announcement document completely disclosed the punishment of the enterprises involved, the accountability results of several managers and the details of the enterprise's rectification and acceptance, which sounded a heavy safety alarm for the cement industry's external construction and the safety management of equipment maintenance in the plant area.
According to the public information, the accident occurred on February 9, 2023, when the high-tech branch of Chengdu Zhongcai Xinjiahao Construction Engineering Co., Ltd. carried out the installation of slag hoist in the factory area of Panzhihua Ruifeng Cement Co., Ltd. and a safety accident occurred during the operation, resulting in one casualty and a direct economic loss of 1.1 million yuan. It is a typical accident of safety responsibility for cooperative construction work outside the plant area. After the accident, the people's government of Panzhihua West District quickly set up a special accident investigation team, adhered to the principle of "four not letting go" safety production investigation, checked the accident process, causes, casualties and property losses in an all-round way, and accurately delineated the safety responsibilities of all parties. On March 31, 2023, the accident investigation report was formally approved by the government.
In order to test the effectiveness of accident handling, verify the implementation of accountability landing, hidden danger rectification and system optimization, the local government, led by the West District Security Office, set up a special evaluation working group with the Bureau of Economy and Information Technology of the District, the Emergency Management Bureau, the West District Branch of the Municipal Public Security Bureau, the District Federation of Trade Unions, the Park Management Committee and other departments. The working group completed the special assessment by formulating a special assessment plan, sorting out and filing massive data, and checking the follow-up disposal of the accident on the spot. During the verification process, it was found that the construction units involved had completed the task of evacuating the factory area, the installation of Ruifeng cement slag hoist equipment had been completed, and the enterprise had completed a series of safety rectification work synchronously.
At the level of accountability, the punishment for this accident is fully covered and in place, without omission or lag. On the enterprise side, Panzhihua West District Emergency Management Bureau imposed an administrative penalty of 650,000 yuan on Chengdu Sinoma Xinjiahao Construction Engineering Co., Ltd. and Panzhihua Ruifeng Cement Co., Ltd. according to law. The penalty decision was issued on May 8, 2023, and the two enterprises paid the fine in full and on time. In terms of
individual accountability, a total of six relevant staff members were punished according to law, covering construction units, management of cement production enterprises, safety management posts and front-line operation management posts. Among them, the legal representative of the outsourcing company and the person in charge of the project were fined 121,700 yuan and 14,700 yuan respectively; Several managers of Ruifeng Cement were held accountable. The general manager of the company, the director of the safety and environmental protection department, the safety supervisor and the workshop safety officer were fined 83,500 yuan, 23,200 yuan, 17,200 yuan and 16,100 yuan in turn. All individual penalties were implemented in May 2023.
In order to thoroughly rectify the potential safety hazards and prevent the recurrence of similar accidents, Ruifeng Cement carried out all-round rectification work for the accident. The enterprise shall organize the safety production warning education for all employees, cooperate with external professionals to carry out the investigation of hidden dangers in the whole plant area, improve the equipment maintenance and maintenance account, improve the safety production responsibility system for all employees, strengthen the investigation and assessment of "three violations" in the plant area, and strictly control the access and training assessment of special operators. At the same time, we should revise and improve the management system of dangerous operations, accurately classify the risks of high-altitude operations, and establish a dual prevention mechanism for safe production.
In view of the common loopholes in outsourcing management in the industry, enterprises strictly implement the examination and approval system for special operations, strictly examine the qualifications of outsourcing units and operators, implement admission safety education, safety technical disclosure, daily on-site inspection and other work, and eradicate the safety management loopholes of "contracting for management, contracting without management". In addition, Ruifeng Cement will include the construction units involved in the blacklist of enterprise safety production, and prohibit them from undertaking all kinds of construction operations within five years. After joint assessment and acceptance by
multiple departments, the accountability for the "2.9" object strike accident has been fully implemented, and the rectification measures such as hidden danger management, system rectification, personnel training and external supervision of the enterprises involved have been effective, the main responsibility of the enterprises for safe production has been effectively compacted, and the overall rectification work has passed the assessment.
