2025, a mechanical injury accident occurred in the plant of China Resources Cement (Changjiang) Co., Ltd. in Chahe Town, Changjiang Li Autonomous County (hereinafter referred to as "China Resources Cement Company"). One person died and the direct economic loss was about 287.

The accident happened at
10:02 on October 22, 2025, after Chen Jiayu and Ji Jianqiang, the rubber coating workers of China Resources Cement Company, completed the processing of the relevant work tickets for the rubber coating of the belt roller. Arrived at the third line at 10:30. Ji Jianqiang was responsible for tying the belt counterweight, Chen Jiayu cooperated, and Chen Mouqin, the head of the loading and unloading section, was responsible for safety supervision.
At 10:40, after wearing the safety belt, Chen Jiayu and Ji Jianqiang went up the stairs to the second floor of the belt counterweight. Chen Jiayu on the south side and Ji Jianqiang on the north side fixed one end of a wire rope on the concrete support seat right above the counterweight with the same round steel, and then connected the other end of the wire rope to a chain hoist respectively. And lower the chain block hook to both sides of the counterweight drum on the first floor (see Fig. 11 below).

The direct economic loss of the equipment and facilities caused by the accident is about 177 yuan. The total direct economic loss of the two items is about 287.2 yuan
. On November 4, the staff entered the site to carry out the detection and identification of the collapse section of the belt corridor. On 7 November, Hunan Sanchang Construction Engineering Testing Co., Ltd. issued the Appraisal Report on the Structure of Axis 1-8 ~ 1-9 (Collapse Section) in Zone A of the Third Line Clinker Conveying Belt Corridor in the Plant Area of China Resources Cement (Changjiang) Co., Ltd. (No.: SC-JD2025 (Qiong) D00001)
. The Appraisal Report indicates the main reasons for the collapse section of the belt corridor: (1) Actual The specifications of the international members are lower than requirements of the original design specifications, resulting in that the bearing capacity of the truss does not meet the requirements of the original design load (this is the main reason for the collapse). Under the original design load, it is calculated that the bearing capacity of 14 members of the truss before collapse does not meet the requirements (as shown by the cloud line in Figure 3-2), and they are the main bearing members (upper and lower chords) of the truss, accounting for 70% of the total number of upper and lower chords of the truss; Among the members that do not meet the requirements, the ratio of load effect to resistance of 9 members is greater than 1.14 (i.e., the ratio of resistance to load effect is less than 0.88, as indicated by "★" in Figure 3-2). According to the 6.3.
(2) of the Standard for Reliability Appraisal of Industrial Buildings (GB 50144-2019), the construction of the joint is not in accordance with the original design, the practice of the fixed hinge end support is inconsistent with the original design, and the central axis of the horizontal support member of the upper chord of the truss does not coincide with the center of the joint. The number of anchoring steel bars of the embedded steel plate at the top of the column is insufficient, the support node has no bottom plate and is directly welded with the embedded steel plate at the top of the column, the anti-disturbance and bearing capacity of the support node are reduced, and the embedded steel plate at the top of the column falls off under the action of long-term load; The construction of the bracing members is not standardized, and the two trusses in the north and south lack of mutual lateral bracing. Under the action of lateral load, the upper chord bearing the axial load in the panel is subject to additional horizontal load, which is not conducive to maintaining the overall stability of the truss.
(3) The structure of the member bar is not constructed according to the original design, and the number of filling plates of the compression double-angle steel member bar is inconsistent with the original design. If the filling plate of the double angle steel compression member is too few and the slenderness ratio of the single angle steel is too large, the single angle steel will lose stability before the whole section under the condition of large compression load, which can not meet the requirements of the original design bearing capacity.
(4) Weld construction quality does not meet the requirements. Node weld surface is generally uneven, discontinuous, and there is overlap, the weld leg size is less than design requirements, and the weld connection does not meet the structural requirements, which violates the principle of "strong node, weak component" in structural design. In the case of large force, the node may be damaged before the member, resulting in the failure of multiple members at the same time and causing collapse.
To sum up, there are major potential safety hazards in the quality of some projects.
Accident cause analysis
1.2. The project quality does not meet the requirements of design and national standards, and the unit (sub-unit) project completion acceptance report that does not meet the actual situation is signed for confirmation, so that the project passes the acceptance smoothly. The hidden danger of engineering quality is the main indirect cause of the accident and bears the main responsibility for the accident.
(2) The project completion acceptance of China Resources Cement (Changjiang) Co., Ltd. is not strictly controlled, and the investigation and management of potential safety hazards is a mere formality. Organize and pass the project completion acceptance under the condition that the survey unit and the design unit have not signed the Unit (Sub-unit) Project Completion Acceptance Report. Hainan Province is a tropical region, with high temperature in summer and more rain and typhoon in autumn and winter, which is easy to accelerate the aging and damage of outdoor equipment. The daily inspection and maintenance are not in place, and the ash and slag of collapsed belt corridor are not in place. Fifteen years after the project was put into use, professional units were not invited to carry out professional inspection and appraisal of the key parts of the belt corridor and concealed works, and the hidden dangers of the project quality were not effectively investigated and treated, which was the primary indirect cause of the accident and the secondary responsibility for the accident.
(3) The supervision unit, Tianjin Jinji Engineering Construction Supervision Co., Ltd., was irresponsible and failed to conscientiously perform its duties of project supervision. Failure to discover and stop the project quality problems in time, signing and confirming the Unit (Sub-unit) Project Completion Acceptance Report that does not conform to the actual situation, so that the project passes the acceptance smoothly, leaving hidden dangers of accidents, is the indirect cause of the accident, and bears the secondary responsibility for the accident. Identification of the nature of
the accident, responsibility identification and handling suggestions
(1) Identification
of the nature of the accident Based on the investigation of the accident, the "10.22" general mechanical injury accident of Changjiang Huarun (Changjiang) Cement Company is a production safety responsibility accident.
(2) Determination
of accident liability 1.
(1) The construction unit, Hebei Building Materials Construction Co., Ltd., violates Note 9 of Article 58 of the Construction Law, Note 13 of Article 28 of the Regulations on Quality Management of Construction Projects, and 5.1.1 and 7.4 of the Steel Structure Design Standard. Article 5.2.7 of the Standard for Acceptance of Construction Quality of Steel Structures (GB 50205-2020), Section 6.3 and 3.
(2) of the Standard for Reliability Appraisal of Industrial Buildings (GB 50144-2019), the project management system of Huarun (Changjiang) Cement Company is not perfect. In the case that the survey unit and the design unit fail to sign the Unit (Sub-unit) Project Completion Acceptance Report, they organize and pass the project completion acceptance in violation of regulations, and the procedures do not meet the requirements. Hainan Province is a tropical region, with high temperature above 37 ℃ in summer and more rainy typhoon weather in autumn and winter, which is easy to accelerate the aging and damage of outdoor equipment. Fifteen years after the project was put into use, no professional inspection and appraisal institution was invited to inspect the key parts and concealed works of the belt corridor, and failed to effectively investigate and control the hidden dangers of the project quality. It was suggested that Changjiang County Comprehensive Administrative Law Enforcement Bureau should be punished in accordance with the provisions of Note 6, Paragraph 1, Article 114 of the Safety Production Law;
(3) Qiqihar North Engineering Construction Supervision Co., Ltd. (Now renamed Tianjin Jinqi Engineering Construction Supervision Co., Ltd.) Is not in place to perform its engineering supervision duties, violating the provisions of Article 35, paragraph 1, of the Construction Law and Note 14 of Article 36 of the Regulations on Quality Management of Construction Projects, and shall bear secondary responsibility for the occurrence of engineering accidents. It is suggested that Changjiang County Comprehensive Administrative Law Enforcement Bureau be punished in accordance with the provisions of Note 6, paragraph 1, Article 114 of the Safety Production Law.
(4) The Emergency Management Bureau of Changjiang Li Autonomous County, the Bureau of Science, Technology and Industrial Informatization of Changjiang Li Autonomous County and the Management Committee of Hainan (Changjiang) Clean Energy High-tech Industrial Park failed to implement the responsibility of comprehensive safety supervision and industry supervision, and failed to supervise and inspect the safety production of China Resources Cement (Changjiang) Co., Ltd. It is suggested that the county leaders in charge of the county government should interview the principal and responsible persons of the relevant units.

(2) Yang Mouzhang, male, born on January 5, 1954, has passed away. The former project manager of the construction unit, Hebei Building Materials Construction Co., Ltd., is the direct person responsible for the construction site. He did not strictly perform his management duties, did not organize the construction in strict accordance with the design documents, and the quality of the project did not meet the design requirements. The quality of part of the project focused on potential safety hazards, and he was responsible for the accident. (Because Yang Mouzhang has passed away, he will not be investigated for relevant legal responsibilities).
(3) Guan, male, 48 years old, ID No.: 360 * * * * * * * * * * 333, general manager and principal responsible person of China Resources Cement (Changjiang) Co., Ltd., failed to perform the safety production duties of the principal responsible person of the enterprise, and was unaware of the hidden dangers of quality and safety of the project without strict acceptance. In violation of the provisions of Article 21 (5) of the Safety Production Law, he bears the main leadership responsibility for the accident. It is suggested that the County Comprehensive Administrative Law Enforcement Bureau, in accordance with the provisions of Note 4, paragraph 1, Article 95 of the Safety Production Law, impose an administrative penalty of 40% of the annual income of the previous year, and report the penalty results to the County Emergency Management Bureau for the record.
(4) Zhang, male, 37 years old, ID No.: 142 * * * * * * * * * * * * * 837, deputy general manager and safety director of China Resources Cement (Changjiang) Co., Ltd., did not supervise and inspect the work of safety production enough, and did not effectively discover and eliminate the hidden dangers of project quality and safety. In violation of the provisions of Article 25 (5) of the Safety Production Law, he bears important leadership and management responsibility for the accident. It is suggested that the county comprehensive administrative law enforcement Bureau, in accordance with the provisions of Note 5 of Article 96 of the Safety Production Law, impose an administrative penalty of a fine of more than 20% and less than 50% of the annual income of the previous year, and report the result of the penalty to the county emergency management Bureau for the record.
(5) Chen Mouqin, male, 33 years old, ID No.: 460 * * * * * * * * * * 61X, chief of the loading and unloading section of the manufacturing department of China Resources Cement (Changjiang) Co., Ltd., the daily organization of safety inspection is not in place, and the investigation and treatment of potential safety hazards in the belt corridor is a mere formality. In violation of the provisions of Article 25 (5) of the Safety Production Law, he bears direct leadership responsibility for the accident. It is suggested that the county comprehensive administrative law enforcement Bureau, in accordance with the provisions of Note 5 of Article 96 of the Safety Production Law, impose an administrative penalty of a fine of more than 20% and less than 50% of the annual income of the previous year, and report the result of the penalty to the county emergency management Bureau for the record.
(6) Gao Moucheng (the legal representative of Tianjin Jinqi Engineering Construction Supervision Co., Ltd., the supervision unit) failed to perform the safety production management duties of the principal responsible person of the enterprise, violated the provisions of Article 21 (5) of the Safety Production Law, and was mainly responsible for the accident. It is suggested that the County Comprehensive Administrative Law Enforcement Bureau, in accordance with the provisions of Note 4, paragraph 1, Article 95 of the Safety Production Law, impose an administrative penalty of 40% of the annual income of the previous year, and report the penalty results to the County Emergency Management Bureau for the record.
(7) Hanmou (former chief engineer of Qiqihar North Engineering Construction Supervision Co., Ltd.), the first person responsible for supervision at the construction site, violated the provisions of Articles 36 and 38 of the Regulations on Quality Management of Construction Projects, signed and confirmed the false Quality Assessment Report. As a result, the project that does not meet the design requirements passes the completion acceptance smoothly, violates the provisions of Article 25 (5) of the Safety Production Law, and bears the leadership responsibility for the accident. It is suggested that the county comprehensive administrative law enforcement Bureau, in accordance with the provisions of Note 5 of Article 96 of the Safety Production Law, impose an administrative penalty of a fine of more than 20% and less than 50% of the annual income of the previous year, and report the result of the penalty to the county emergency management Bureau for the record.
Accident lessons and preventive measures
accident units should clarify the responsibilities of production and operation units, firmly establish the concept of safety development, adhere to the principle of "safety first, prevention first, comprehensive management", draw lessons from accidents, implement the principle of "four not let go", and earnestly do a good job in future safety production. In future production and operation activities, we must strictly abide by the provisions of laws, regulations and rules such as the Safety Production Law, implement the main responsibility of safety production, and conscientiously do a good job in safety production. In order to prevent and eliminate the occurrence of similar production safety accidents and ensure production safety, the following preventive measures and suggestions are put forward:
(I) Hebei Building Materials Construction Co., Ltd.
shall establish a sound quality assurance system in strict accordance with the provisions of the Construction Law and the Regulations on Quality Management of Construction Projects. The construction shall be carried out in strict accordance with the approved construction drawing design documents and construction technical standards. The building materials, components, fittings and equipment entering the site must be inspected, and those that fail to pass the inspection shall not be used. The quality inspection and record of concealed works must be done well, and the construction and supervision units shall be notified for acceptance.
(2) China Resources Cement (Changjiang) Co., Ltd.
shall strictly comply with the relevant requirements of the Regulations on Quality Management of Construction Projects, the Regulations on Acceptance and Acceptance of Completion of Housing Construction and Municipal Infrastructure Projects, and the Measures for Examination and Management of Construction Drawing Design Documents of Housing Construction and Municipal Infrastructure Projects. Improve and implement the internal engineering quality management system of construction projects, entrust units with corresponding qualifications to carry out construction and management, strictly implement the construction drawing design and review system, especially strictly perform the organizational responsibilities of completion acceptance, ensure that the procedures are legal, the contents are true, the conclusions are accurate, take overall responsibility for the quality of the project, and unqualified projects shall not be put into use. Increase the investment in safety production funds, materials, technology and personnel, and formulate a detailed Daily Safety Checklist for different facilities such as belt corridor, raw material warehouse, clinker warehouse and high-altitude passage, including structural parts (welds, bolts, rods), protective facilities, operation status (abnormal sound, vibration), anti-corrosion status, etc. Strengthen personnel training and special operation management, establish periodic appraisal, strengthen daily management and control, form closed-loop management of industrial building safety risk, prevent the recurrence of similar collapse accidents, and ensure the safety of personnel life and property.
(3) Qiqihar North Engineering Construction Supervision Co., Ltd.
shall improve and implement the supervision and management system of construction projects in strict accordance with the relevant requirements of the Regulations on Quality Management of Construction Projects and the Standards for Supervision of Construction Projects, and in accordance with laws, regulations, technical standards, design documents and supervision contracts. The whole process supervision of construction quality is carried out in the form of side station, patrol inspection and parallel inspection. On-site supervision must be carried out for the construction of key parts and key processes. The quality assurance system of the construction unit must be reviewed, the materials entering the site must be verified, and the inspection lot, subdivisional and divisional works must be accepted and signed. The appraisal report issued must be true, objective and accurate.
(4) The supervisory departments
of the industry need to further strengthen the supervision and guidance of the safety production of enterprises, and promote enterprises to actively establish and improve the dual prevention mechanism of safety risk control and hidden danger investigation and management. At the same time, in strict accordance with the provisions of the Safety Production Law, the Construction Law and the Regulations on Quality Management of Construction Projects, we should strengthen the supervision and management of the whole process of construction, especially the supervision and law enforcement of the completion and acceptance links.
