The "3 · 6" general mechanical injury accident in the mixing station of Chenshi Street Component Factory in Yongchuan District of Chongqing is a production safety responsibility accident caused by workers' serious violation of safety operation rules. At the same time, the accident investigation team put forward suggestions on the handling of the accident responsible units and relevant responsible personnel, as well as suggestions on accident prevention and rectification measures. I. Basic information
(I) Overview of the unit where the accident occurred.
Sichuan Litian Construction Labor Service Co., Ltd., legal representative: Chen, registered capital: RMB two hundred and thirty million, company type: limited liability company (sole proprietorship of natural person), establishment date: June 28, 2020, business scope: apartment and villa engineering construction activities; indemnificatory engineering construction activities; residential housing construction; Construction labor subcontracting; construction labor subcontracting; sales of building materials, etc. (For projects that must be approved according to law, business activities can only be carried out with the approval of relevant departments).
The Company has obtained the Work Safety License issued by the Department of Housing and Urban-Rural Development of Sichuan Province. The scope of the license is construction, and the validity period is from October 8, 2023 to October 8, 2026.
The Company has obtained the Construction Enterprise Qualification Certificate issued by the Housing and Urban-Rural Development Bureau of Luxian County, Sichuan Province, with the validity period of September 9, 2025, and the qualification category and grade: construction services are not graded.

(IV) Accident process. At about 18:10 on March 6,
2024, Liu, Wang and other workers of Sichuan Litian Construction Labor Co., Ltd. were cleaning up the commercial concrete mixing station. At about 18:15, Wang drove a loader to clean up the spilled slag under the old hopper at Station 1. Liu asked if he needed help. Wang asked Liu to go to the dressing room to change clothes and gather in the central control room to prepare for the cleaning of the pot. At about 18:30, Wang cleaned up the slag, returned to the central control room and waited for Liu, and reported to the on-site security officer Liu. Liu immediately went to the office to prepare safety education materials and related safety equipment for pot cleaning operations. At 18:39, Wang saw that Liu had not returned from changing clothes, so he started the machine to check whether there was any residue. After starting the machine, Wang heard the abnormal noise, immediately shut down the machine, rushed to the 3rd floor platform for inspection, and found that the No.1 mixing pot manhole had been opened, Liu had no vital signs in the pot, and then reported the situation to the stationmaster Lv. After receiving the report, Lv Mou and Geng Mou, the safety director of the building component factory, rushed to the scene of the accident immediately and found that Liu Mou had no vital signs.
(Five) the scene of the accident. The site of the
accident is the mixing station of Chenshi Street Component Factory in Yongchuan District of Chongqing City. The scene of the accident is the mixing pot of No.1 production line on the third floor of the mixing station. The inspection port of the mixing pot is open. There is a pneumatic pick at the benchmark beside the mixing pot. The key of the operation platform of the central control room of the mixing pot is still inserted in the control platform. The warning sign of "No closing" is placed on the top of the air conditioner beside the control platform.

The accident resulted in one death. The name of the deceased: Liu, male, Han nationality, 55 years old, is a worker of Sichuan Litian Construction Labor Co., Ltd.
Direct economic loss: 2.08 million yuan.
2. Emergency disposal and assessment
of accidents (1) Reporting and response of accident information. 58 on March 6,
2024, after the District Emergency Management Bureau received the accident report from Chen Shi Street Office, the relevant personnel of the District Emergency Management Bureau rushed to the scene of the accident immediately, checked and inspected the accident, and issued the "Decision on On-site Treatment Measures" to stop the production line No.1 of the mixing station. After receiving the police, Chen Shi police station arrived at the scene of the accident at about 19:30 to carry out a preliminary investigation of the accident.
(2) Emergency disposal at the scene of the accident. At about 18:40 on March 6,
2024, worker Wang Mou found that Liu Mou had no vital signs in the mixing pot of No.1 production line, and immediately reported to the stationmaster Lv Mou. After receiving the report, Lv Mou rushed to the scene of the accident with the safety director of the component factory, and found that Liu Mou had no vital signs, and immediately started the contingency plan. Arrange Geng Mou, Liu Mou and others to guard, evacuate other workers, protect the scene of the accident, arrange Liu Gaofeng and others to consult online information, conduct public opinion monitoring, Lv Mou dialed 110 alarm calls, and arrange Geng Mou to report the accident to Chen Shi Street Emergency Office.
(3) The handling of the aftermath. After the
accident, Lv Mou, the stationmaster of the mixing station, immediately started the contingency plan, set up a team to deal with the aftermath of the accident, notified the family members of the deceased to do a good job of appeasing, and signed the Agreement on Reconciliation of Work and Death with the family members of the deceased that evening, compensating the family members of the deceased for a total of 2.08 million yuan, and the family members of the deceased were emotionally stable without adverse social impact.
(4) Assessment of emergency response to accidents. After the
accident, the accident unit immediately started the contingency plan, protected the accident scene, controlled the public opinion of the accident, dialed 110 alarm telephone, reported the accident according to the regulations, and actively did a good job in appeasing the family members and dealing with the aftermath, without causing adverse social impact. In this accident, the accident unit can effectively organize emergency disposal, and the effect is good.
3. Analysis of the cause and identification
of the nature of the accident (1) The direct cause of the accident. Liu Mou, a
pot cleaner, seriously violated the safety operation rules of the pot cleaner, did not receive pre-cleaning education (safety disclosure), did not cut off the main power supply of the motor of the control cabinet, did not remove the key from the control cabinet for safekeeping, did not place a warning sign for inspection and maintenance, did not disconnect the contact limit switch of the mixing pot, and then entered the cylinder of the mixing pot for cleaning. It is the direct cause of the accident.
(2) Indirect causes of the accident.
1. The workers of Sichuan Litian Construction Labor Co., Ltd. have weak safety awareness and seriously violate the safety operation rules of the mixing pot to venture into the mixing pot cylinder for cleaning operation, which reflects the inadequate safety education and training for workers.
2. Sichuan Litian Construction Labor Co., Ltd. has not carried out effective safety supervision on dangerous operations in limited space, and has not timely detected and stopped workers'serious violations of safety operation rules and risky operations.

4. The performance of the
supervisory units in the Chenshi Street Office of Yongchuan District, Chongqing, has set up an emergency office to implement comprehensive safety supervision, and the Economic Development Office to implement industry safety supervision. The street has formulated the "Chen Shi Street 2024 Safety Production Supervision and Inspection Plan" (Yong Chen Shi Ban Fa [2024] No.1), which includes the building component factory of Chongqing Urban Construction Development Co., Ltd. in the annual supervision and inspection plan. On February 19, 2024, the factory applied for resumption of work and production, and the Development Office reviewed the relevant information of the factory and agreed to resume work and production. On January 25, February 23 and March 4, 2024, the street organization held three safety production work meetings, and notified the component factory to participate in the meeting; on March 14, the street hired experts to carry out a comprehensive safety inspection of the component factory, completed the March supervision plan, fulfilled the territorial safety supervision responsibilities, and suggested not to investigate the responsibility.
5. The main problems
existing in the units responsible for the accident are that Sichuan Litian Construction Labor Service Co., Ltd. has failed to supervise the implementation of the Safety Regulations for Limited Space Operations in Industrial and Trade Enterprises, failed to supervise the safety of dangerous operations in limited space, failed to educate and train workers on safety, and workers'safety awareness is weak.
