At 11:30 on May 28, 2026, in the clinker branch of Qinghai Huzhu Jinyuan Cement Co., Ltd. in Donghe Township, Huzhu County, Haidong City, Qinghai Province, a fall accident occurred during the construction of the steel structure sealing project of the expansion of the raw material stacking shed by the workers of Hangzhou Wanda Steel Structure Co., Ltd. One person died and the direct economic loss of the accident was 2.227 million yuan. On the day of
the accident, four workers carried out the paving operation of color steel tiles on the roof of the shed. Peng Hua, the worker, drilled holes in the rainwater trench about 50 centimeters wide on the roof of the old raw material shed No.5. The safety protective rope on the roof of the shed on the north side of the site was not laid, and there was no protective net and other anti-falling facilities. Peng Hua illegally stepped on the damaged color steel tile area on the roof of the shed and fell to the ground from a height of 13 meters after losing protection. I. The direct cause

2. Indirect causes
of the accident 1. The contracting unit, Qinghai Huzhu Jinyuan Cement Co., Ltd., is seriously absent from overall management and control. No special management system for outsourced works has been established, no pre-job safety training has been carried out for construction personnel, and training files have been kept; the clause "Party B shall be solely responsible for all safety accidents" has been set in the construction contract, and the legal supervision responsibility for safety production has been transferred in violation of regulations; the safety technical disclosure data is missing and the content is not compliant; The special construction scheme and supervision data are not perfect, the contractor's safety management system has not been audited for compliance, and the major risk prediction and control of high-altitude operation on the old shed roof is insufficient.
2. The main responsibility of the contractor Hangzhou Wanda Steel Structure Co., Ltd. for safety production has not been fulfilled. The key personnel of the project did not comply with the appointment and assessment procedures, and the person in charge of the project was off duty without authorization on the day of the accident; the safety production responsibility system of the whole staff was not established, and the post responsibility statement was not signed; the core safety management systems such as labor insurance, special operation and safety expenses were missing; the safety production related liability insurance was not insured, and the contingency plan was not audited and the drill was not organized; The three-level safety education, pre-shift disclosure, safety technical disclosure and hidden danger investigation account are all blank, and the site safety management system is invalid.
3. Suggestions on
responsibility handling 1. Personnel disposal: Peng Hua, the deceased, has died and is exempted from accountability; For the legal representative of the contract-issuing party, the deputy general manager, the director of the clinker factory, the person in charge of safety and environmental protection, the project manager of the contractor, the person in charge of the site and the supervisor of the site, a total of 7 relevant responsible persons, the administrative penalty suggestions for the corresponding proportion of wage fines are put forward in accordance with the Safety Production Law.
2. Unit disposal: administrative punishment suggestions were made to Qinghai Huzhu Jinyuan Cement Co., Ltd. and Hangzhou Wanda Steel Structure Co., Ltd.; relevant clues of Hangzhou Wanda Steel Structure Co., Ltd. failing to pay industrial injury insurance were handed over to the comprehensive law enforcement department for handling.
3. Supervision units: the County Housing and Urban-Rural Development Bureau, the County Industry and Information Technology Bureau, and the Donghe Township Government all implement the daily safety production deployment, hidden danger investigation and other supervision responsibilities in accordance with the regulations, and the performance information is complete, so the relevant responsibilities will not be investigated.
