Resulting in one death! Safety accidents in cement products enterprises

2025, in the mixing workshop of Taizhou Luyou Building Materials Co., Ltd. (Hereinafter referred to as "Luyou Building Materials"), located in Yanjiang Town, Linhai City, a tire loader had a vehicle injury accident on the way to recover the remaining mixed raw materials. As a result, a workshop handyman was crushed on the spot and died after being sent to the hospital for rescue.

Luyou Building Materials is a building materials manufacturing enterprise mainly engaged in the manufacturing and sales of lightweight building materials, cement products and concrete structural components, with a registered capital of 5.8 million yuan and 20 employees. Sun Gende, the legal representative, is the main person in charge of the company, and Ma Lili is the full-time safety production management personnel. The core production process of the

enterprise is the mixing of sand and gravel raw materials and the production of asphalt concrete. After the delivery of asphalt concrete on the day of the accident, the loader recovers the remaining asphalt-free mixed raw materials in the hopper of the mixing workshop according to the instructions and transfers them to the raw material yard for storage. The vehicle involved is a LW500HV tire loader manufactured in 2019, with a rated load of 5 tons. The driver, Lin Xiaoming, holds a legal and valid in-factory motor vehicle operation certificate and is qualified for employment. At the time of the

accident, the driver Lin Xiaoming drove the loader to complete the loading and turned from the workshop to the raw material yard. Wang Yide, a handyman in the workshop, entered the loader operation area illegally and happened to be in the blind area of the driver's vision. After being knocked down by the right side of the loader hopper, he was crushed by the right tire, resulting in tragedy.

After the accident, the director of the workshop at the scene signaled the driver to stop at the first time. Sun Gende, the principal person in charge of the enterprise, rushed to the scene immediately, organized workers to carry out cardiopulmonary resuscitation first aid, and synchronously dialed 120 emergency calls and 110 alarm calls. The injured were then sent to the First People's Hospital of Linhai City for emergency treatment. They died of crushing injuries at 16:30 on the same day. The deceased, Wang Moude, 54 years old, from Anhua, Hunan Province, was a handyman in the company's workshop. After mediation by the People's Mediation Committee

of Yanjiang Town, Luyou Building Materials reached a compensation agreement with the family members of the deceased, and paid a total of 1.1 million yuan in one-time compensation for industrial death and funeral expenses, which was paid in full on November 26, 2025. The aftermath disposal work was smooth and orderly, and the emergency disposal process was assessed by the investigation team in a timely manner. The cause of the

accident was identified

by the investigation team after comprehensive verification, excluding their own diseases, intentional injuries and other factors, and it was clear that the accident was a general production safety liability accident, and the direct and indirect management loopholes were clear.

Direct cause: The deceased Wang Moude, as a workshop handyman, had extremely weak safety awareness and seriously violated the company's "Safety Operation Rules for Cleaning Personnel". During the normal operation of the loader, he entered the designated dangerous area of operation in violation of regulations and was in the blind area of the driver's sight. Finally, he was knocked down and crushed by the loader and died. He was mainly responsible for the accident because he had died. The relevant responsibilities will no longer be investigated.

Indirect core problem: the main responsibility of safety production of Luyou building materials is seriously suspended, and the system exists in name only. Firstly, the implementation of safety system is not in place, education and training are mere formalities, the main person in charge does not participate in safety training according to regulations, the loader operation area does not set up obvious safety warning signs, the risk control of cross-operation is completely missing, and the major hidden dangers of cross-operation between vehicles and personnel are not timely investigated; Second, the performance of post responsibilities is not in place, the company has not formulated special loader safety operation rules, the main responsible person, safety management personnel have not fulfilled the legal responsibilities of hidden danger investigation and management, risk management and control, safety management is nothing. According to the division of responsibility for the accident, the

investigation team put forward clear suggestions for the relevant responsible units and personnel. Firmly consolidate the safety responsibility:

Enterprises involved: Luyou Building Materials has a number of violations of laws and regulations, such as lack of safety training, lack of warning signs, inadequate investigation of hidden dangers, and inadequate risk control, and is directly responsible for the accident. It is suggested that the emergency management department impose administrative penalties according to law. Sun Gende,

the main person in charge, said that he had not fulfilled his statutory duty of safe production, had not formulated special operating rules for loaders, had not implemented a dual prevention mechanism, and was responsible for the accident. He suggested that administrative penalties be imposed according to law. Ma Lili, the

safety management personnel, failed to perform the duties of the safety management post, did not participate in the formulation of equipment operation procedures, did not timely investigate the hidden dangers of the accident, and was responsible for the accident.
